Intake Paperwork
You’ll receive an email with intake forms to complete before your first session. Completing them in advance allows us to focus more fully on your goals.

Appointment Confirmation
You’ll receive confirmation of your session’s date, time, and format (in-person or virtual).

In-Person Sessions
Please arrive 15–20 minutes early, as additional paperwork may be required on-site.

Virtual Sessions
Ensure you’re in a private, quiet space with a strong internet connection. A secure video link will be provided in advance. Logging in a few minutes early is recommended.

What to Provide Before Your First Session
Please submit the following items prior to your appointment:

  • Completed intake and assessment forms, including signatures and dates

  • Insurance information, including provider name, member ID, and a copy of the front and back of your insurance card.

  • A valid photo ID, including copies of both the front and back

  • Any relevant medical or mental health records, if applicable, for insurance or clinical purposes

Insurance Coverage
Patients are responsible for verifying their mental health benefits prior to the first session. Please contact your insurance provider directly to confirm coverage, copays, deductibles, and whether pre-authorization is required. Be sure to request and provide the therapist with the reference or case number from your call for documentation purposes.

Cancellation Policy
Please provide at least 24 hours’ notice for cancellations or rescheduling. Missed appointments or late cancellations will be charged the full session fee.


WHAT TO EXPECT IN YOUR FIRST SESSION

Beginning therapy while navigating the challenges of cancer, chronic illness, or a rare diagnosis can bring up a range of emotions—curiosity, hope, uncertainty, or even anxiety. This is completely normal. You don’t need to have everything figured out—just come as you are. Our time together is a space for openness, safety, and support at your own pace.

In our first session, we’ll focus on getting to know each other. I’ll ask questions to better understand your background, current concerns, and what you hope to gain from therapy. Some questions will also help meet any insurance or documentation requirements. Whether you’re seeking individual support or coming as a family, you’re encouraged to share what feels important and ask any questions about the process.

If you feel comfortable moving forward, we’ll schedule our next session and begin working together toward your goals—whatever healing, clarity, or connection looks like for you in this season.


SESSION LENGTH AND FREQUENCY

  • Individual Sessions: 50 minutes

  • Couples and Family Sessions: 75 minutes

  • Extended Sessions: Available upon request, based on clinical need and scheduling availability

Session Frequency
Sessions are typically held once per week; however, bi-weekly or more intensive sessions may be arranged based on your goals and therapeutic needs. We'll collaborate to create a schedule that supports your progress and fits your lifestyle.

Telehealth Availability
I offer secure Telehealth sessions for patients located within the state where I am licensed. Please note that, due to licensing laws, patients must be physically present in that state at the time of the session—even when meeting virtually.

While I may occasionally be out of the country, I continue to offer Telehealth services during those times, as long as you remain within the state of my licensure. If you plan to travel or relocate temporarily, please let me know so we can discuss available options and ensure continuity of care.

Please note: I do not work with children, as my clinical setting is not designed to accommodate pediatric populations.


PAYMENTS AND FEES:

  • Individual Therapy: $225 per session

  • Couples & Family Therapy: $285 per session

  • Sliding Scale: Available based on need and limited availability

Insurance
I am an in-network provider with:

  • Blue Cross

  • Kaiser

If you plan to use insurance, please verify your mental health benefits before scheduling your first therapy session. Once confirmed, provide your case number or reference call number from your insurance provider for my records to ensure coverage for mental health services. Patients are responsible for any co-pays, deductibles, or services not covered by their plan.

Late Payments & Returned Checks
A $25 fee will be applied for returned (bounced) checks.
Late payments may be subject to additional fees or interruption of services. Please communicate any payment concerns in advance so we can discuss options.
